Make A Choice is proud to partner with organizations, health systems, community groups and others who educate our community members and help us promote the Make A Choice initiative.
Make a Choice Program Delivery Partners work directly with the populations to educate them about type 2 diabetes prevention, chronic disease self-management, chronic pain management, and arthritis self-management. They are health organizations, hospitals, community centers and much more!
Do you or your organization lead Diabetes Prevention Program (DPP), Diabetes Self-Management Programs (DSMP), Walk with Ease (WWE) or similar chronic health or chronic disease programming? Would you like to partner with Health Promotion Council to receive referrals?
